Thank you very much for participating in the End-Of-Program Questionnaire. This will help us improve our program for future students like you. Before you begin, a few important reminders:

• You have 30 days after the end date listed on your DS-2019 form in which you can remain in the U.S. to travel. If you leave the U.S., you will not be allowed back into the U.S. after the end date listed on your DS-2019 form.
• If you are insured through ERDT, your insurance covers you for 4 months after your date of arrival. If you plan to stay longer than 4 months, please contact ERDT to purchase an additional month of insurance.
• Students remaining in the U.S. for more than 30 days after the end date listed on their DS-2019 form are subject to arrest and deportation. The Work & Travel Program cannot be extended.
